(II) housing advice and outreach service for adults aged 25+ — delivery of a specialist housing advice and assistance service to adults aged 25+ who are rough sleeping, homeless or at risk of becoming homeless in line with homelessness legislation. This includes the provision of flexible, bespoke and outcome focussed advice and outreach services to support individuals to access and maintain suitable accommodation, promote healthier lifestyles choices and, working with relevant statutory and voluntary partners, address wider support needs. The service addresses the causes and consequences of homelessness and aims to both prevent rough sleeping and ensure that those who are sleeping rough are supported off the streets. The service also positively manages the transition of these individuals into accommodation and supports the delivery of the Solihull Housing First service through.
